How it Works:

Submit Your Form
Fill out the form below and answer all the questions to the best of your ability.

Convenience Fee
You will receive a link or a phone call to pay the $100 convenience fee required. This fee will be applied to your surgery if you choose to move forward.

Set Up Your Account
You will receive an email with two documents that provide a step-by-step guide on creating your account on Nextech, our HIPAA-compliant electronic medical record system, and how to upload your photos.

Upload Your Photos
Once your account is created and your photos are uploaded, just sit back and wait!

Review and Quote
Dr. Rodriguez will review your information and photos. Your candidacy for surgery, a tentative surgical plan, and a quote will be sent straight to your email.

Schedule Your Surgery
If you are ready to move forward with surgery, our patient care coordinator will help schedule your in-person consultation and consent appointment for surgery.
